The undersigned requests that a service line be installed from the water main to the property line and a meter be set for the lot designated below:
The undersigned also agrees to pay to the District all expenses for the installation of the water service and for setting the meter for the property designated above including labor, equipment charges, material (including meter and meter setter), paving costs, road opening fees and any other costs associated with the installation. The undersigned agrees that the installation and use of the water is subject to the rules and regulations of the Sanford Water District and further agrees that the service line and meter shall be located and sized in a manner suitable to the District.
